What is Ellansé M?

Ellansé M is an advanced polycaprolactone (PCL)-based tissue biostimulator manufactured by Sinclair Pharma — one of the longest-used and most extensively studied biostimulaors on the aesthetic medicine market. The product combines immediate volumetric action from its CMC gel carrier with long-lasting biostimulation of neocollagenogenesis by PCL microspheres. The effect lasts up to 2 years — hence the designation M (for "medium"), distinguishing it within the Ellansé family (S/M/L/E).

Thanks to full integration with the extracellular matrix, the product is virtually undetectable under the skin — both at rest and during facial expression (speaking, laughing, crying). The complete biodegradability of its components eliminates the risk of allergic reactions and granuloma formation.

Two-stage mechanism of action of PCL

Ellansé works in two stages — which distinguishes it from both classic HA fillers (mechanical effect only) and PLLA (no immediate effect):

• Stage 1 – immediate filling (first weeks): the CMC (carboxymethylcellulose) gel carrier fills wrinkles and volume deficits immediately after injection — the effect is visible directly after the procedure. After approximately one month the CMC gel is gradually absorbed, which may cause a partial reduction in the achieved volume. This is a normal stage of the process.
• Stage 2 – neocollagenogenesis (subsequent months): PCL microspheres larger than 20 µm with a perfectly smooth surface undergo encapsulation by the surrounding tissue — the body forms a capsule around them, creating a biological scaffold along which fibroblasts deposit new type I collagen fibres. Skin density, elasticity and quality all improve. The face not only regains lost volume but also undergoes significant structural rejuvenation.

Crucially, PCL microspheres are not absorbed by macrophages (no phagocytosis) thanks to their size exceeding 20 µm — throughout their presence in the tissue they actively stimulate collagen synthesis before biodegrading themselves.

The Ellansé range – S, M, L, E – what are the differences?

All Ellansé variants contain the same composition (70% CMC + 30% PCL), differing only in duration of results, which depends on the properties of the PCL microspheres and their rate of biodegradation:

• Ellansé S — approx. 1 year. For patients trying the treatment for the first time or with shorter-term expectations.
• Ellansé M — approx. 2 years. The most popular variant — a balance between duration of results and flexibility in case patient preferences change.
• Ellansé L — approx. 3 years. For patients certain of the desired result, seeking a longer-term solution.
• Ellansé E — approx. 4 years. The longest-acting variant, for demanding patients with significant volume loss.

How does Ellansé differ from HA filler and PLLA biostimulator?

• HA filler — purely mechanical filling of tissue space and hydration. No stimulation of own collagen. Results last 6–18 months and disappear completely after HA biodegradation. Easily reversible (hyaluronidase).
• PLLA (poly-L-lactic acid, e.g. Sculptra) — synthetic biostimulator with no immediate effect. Requires several sessions, effects develop slowly (4–12 weeks). Risk of nodules with incorrect technique. Results last 2–3 years.
• Ellansé M (PCL) — combines the best of both: immediate filling (CMC) and lasting stimulation of the patient's own collagen (PCL). One session is sufficient for a comprehensive result. No nodule risk thanks to perfectly smooth microspheres. Results last 2 years. Full biodegradation with no residue.

Treatment procedure and recovery

The product is injected using a needle or cannula. The filling effect (CMC) is visible immediately after the procedure. After approximately one month, a partial reduction in achieved volume occurs — this is a normal and expected stage caused by CMC degradation. Over the following 2–6 months, gradual collagen synthesis takes place — the face not only regains lost volume but skin quality also significantly improves.

Recovery is brief. In most cases patients can resume daily activities immediately after the procedure. Some patients may experience localised redness, swelling or bruising (if a vessel is affected), resolving within a few days.

Post-treatment care

FAQ – questions and answers

Why does the effect partially reduce after one month?
This is a normal and expected stage of how Ellansé works. The CMC gel (carrier) provides immediate filling but is absorbed after approximately 4 weeks — this is a transient reduction in volume. Simultaneously, PCL microspheres gradually stimulate the body's own collagen synthesis, which compensates for the CMC loss and over the following months restores — and even enhances — the result. Patients should be informed of this before treatment to avoid unnecessary concern.

How does Ellansé M differ from S, L and E?
All variants have the same composition — they differ only in duration of results: S (1 year), M (2 years), L (3 years), E (4 years). Ellansé M is the most commonly chosen variant as a golden mean — a long enough result while retaining flexibility in case patient preferences change.

Can Ellansé be removed like an HA filler?
No — PCL cannot be deactivated with hyaluronidase (as HA can). The product biodegrades naturally over approximately 2 years. This is important information to convey to the patient before treatment. Correct patient selection and technique are therefore essential.

How many sessions are needed?
Usually one session is sufficient to achieve a comprehensive result. In cases of significant volume loss, a complementary session after 3–6 months may be considered, once the full collagenogenesis effect is already visible.

Is Ellansé suitable for lip augmentation?
No — Ellansé is not indicated for lip augmentation. The product is used exclusively in deeper facial tissue areas (cheeks, jawline, chin, nasolabial folds, nose). Dedicated HA fillers are used for lips.

Can Ellansé be combined with botulinum toxin or HA filler?
Yes — Ellansé combines well with botulinum toxin and HA fillers as part of a comprehensive rejuvenation protocol. Botulinum toxin relaxes the facial muscles, HA fills fine lines, and Ellansé restores volume and stimulates collagen in the deeper layers. A 2-week interval between procedures is recommended.
